RADIOPHARMACEUTICAL: Tc-99m Disofenin or Choletec
ADULT DOSE: 5 mCi IV
PATIENT PREP: NPO 4 hours prior to exam, no meds 4 hours prior to exam
EQUIPMENT & ENERGY:
Dynamic parameters - Planar Imaging
- LEHR collimators
- Matrix 256 x 256
- Zoom 1.23
- Camera peaked at Tc-99m with 15% window
- Frames - 63
- Time - 60 seconds per frame
Static Parameters
- LEHR collimators
- Matrix 256 x 256
- Zoom 1.23
- Detector - 1
- Camera peaked at Tc-99m with 15% window
- 5 minutes or 500 Kcts per image
SCAN PROTOCOL:
- Patient void before scan
- injected with radiopharmaceutical IV
- All external metal objects removed prior to scan
- Lay supine, head facing out, area of interest centered under camera
- Tech to start dynamic images (if pt unable to cooperate, static imaging to be completed up to 1 hr)
- After dynamic imaging, perform right lateral static image
- If gallbladder and small bowel visualized exam is complete
- If gallbladder and small bowel NOT visualized - perform delayed static images at 90 min & 120 min post injection
- At 120 min (2h) post injection if gallbladder NOT visualized but small bowel is visualized, exam is complete
- If gallbladder and small bowel NOT visualized at 120 min (2h), perform delayed static anterior and right lateral imaging at 4 h post injection
- Continued delayed images may be performed at the discretion of the radiologist
PROCESSING:
- Adjust acquired images for optimal viewing
- Label images
- Send images PACS
INDICATIONS: Cholecystitis, bile leak, biliary obstruction, biliary malformation, biliary fistula, patency of biliary-enteric bypass
